How a Podiatrist, Primary Care Doctor, and Vascular Specialist Work Together to Save a Limb

Key Takeaways:

A diabetic limb salvage team brings together a podiatrist, your primary care doctor, and a vascular specialist so wounds, blood sugar, and circulation are treated as one problem instead of three. Coordinated handoffs — wound care, infectious disease, vascular imaging, and revascularization — catch threats earlier and dramatically reduce the risk of a major amputation. Knowing each provider's role and tracking a few key numbers between visits lets you protect a limb before a small wound becomes a hospital admission.

If you have diabetes and have ever had a foot ulcer, an infection that wouldn’t clear up, or other problems, you’ve probably noticed how it’s not just one doctor concerned with your care. A podiatrist watches the wound, your primary care doctor monitors blood sugar and overall health, and a vascular specialist may be brought in to evaluate blood flow. When these three providers work as a team, the odds of saving a limb go up substantially.

At Sunshein Podiatry in Centerville, our Centerville podiatry team partners closely with primary care offices and vascular practices throughout the Dayton area. Patients who lean on this kind of coordinated approach are far less likely to face amputation than patients trying to manage everything alone.

Table of Contents

What Is a Diabetic Limb Salvage Team?

The Three Core Roles That Coordinate Diabetic Foot Care

The Podiatrist Leads Daily Wound and Foot Care

Your Primary Care Doctor Manages the Big Picture

The Vascular Specialist Restores Blood Flow to the Foot

How Providers Work Together on Patient Care?

What Should I Bring or Track Between Appointments?

Why Team-Based Diabetic Foot Care Reduces Amputations

What Is a Diabetic Limb Salvage Team?

A diabetic limb salvage team is a small group of providers who work together as a team to treat a patient when the leg or foot is in danger of amputation.

Diabetes attacks the foot from several directions at once. High blood sugar damages nerves, narrows arteries, and slows the immune system's response to infection. When a podiatrist, primary care doctor, and vascular specialist communicate regularly about patient care, problems can be treated when they’re small – and before they result in infection or gangrene.

Research from the American Diabetes Association's foot complications guide shows that more than 80 percent of diabetes-related amputations are preceded by a foot ulcer. Early multidisciplinary care can prevent the majority of those amputations. A team-based diabetic foot care plan is the difference between watching a small wound on the heel and leaving the hospital without a foot.

The Three Core Roles That Coordinate Diabetic Foot Care

The Podiatrist Leads Daily Wound and Foot Care

A podiatrist is usually the first provider to see a diabetic foot problem and monitors the patient’s wound weekly. Podiatrists debride dead tissue, manage infection, fit offloading devices to take pressure off ulcers, and check sensation, skin, and bone for early signs of breakdown. We also screen for nerve damage at every visit so a patient with reduced sensation knows what to watch for at home.

When advanced wound care, biomechanics, or surgery is needed, our podiatrists oversee that piece of the plan, including foot and ankle surgery when reconstruction is necessary to close a wound or correct a deformity that keeps causing ulcers.

Your Primary Care Doctor Manages the Big Picture

Your primary care doctor (PCP) manages your diabetes care. They monitor your A1C, blood pressure, kidney function, cholesterol, and medications, all of which influence whether a wound will heal. A PCP can also screen for early kidney disease and heart disease, two conditions that change how aggressively a podiatrist or vascular specialist needs to act on a foot problem.

The Vascular Specialist Restores Blood Flow to the Foot

Vascular specialists (either vascular surgeons or interventional cardiologists) are doctors who specialize in blood flow. They focus on the arteries that feed the leg and foot. Vascular specialists use ultrasound, ankle-brachial index testing, and angiography to confirm whether peripheral artery disease (PAD) is starving the wound of oxygen. When circulation is too poor for healing, they perform revascularization procedures (angioplasty, stenting, or bypass) to open the vessels back up. Without restored blood flow, even excellent wound care may not be enough.

How Providers Work Together on Patient Care?

A patient walks in with a small ulcer on the bottom of the foot. The podiatrist debrides it, swabs for bacteria, and orders an X-ray. Then, they might check the blood flow to the foot. If the pulse in the foot is weak, the podiatrist sends a same-week referral to a vascular specialist for imaging. If the wound looks infected or systemic symptoms appear, the PCP is looped in for antibiotics, lab work, and possibly an infectious disease consultation. Each provider sees the same notes, the same wound photos, and the same imaging The patient doesn’t have to track down records for each doctor because they have access to the entire file.

If revascularization is needed, the vascular specialist performs the procedure and then confirms with the podiatry team that blood flow has been restored. From there, the podiatrist can pursue more aggressive wound care, offloading, and skin substitutes or grafts because the tissue finally has the blood flow it needs to heal.

What Should I Bring or Track Between Appointments?

While your care team has access to your files, having certain documents on hand can make conversations with each provider easier.

Keep a one-page list of your medications, doses, and recent A1C and kidney lab values to share with every provider.

Take a daily photo of any wound or open area, with the date in the photo, so changes are easy to spot.

Record temperature differences between feet, swelling, drainage color, and any new pain or numbness.

Bring the actual shoes you wear most days so the podiatrist can check pressure points and recommend changes.

Note what you can feel on the bottom of each foot — and what you cannot — to help track neuropathy over time.

Why Team-Based Diabetic Foot Care Reduces Amputations

Team-based care matters because a diabetic foot problem rarely has a single cause. A wound on the bottom of the foot may be the result of neuropathy, a poor-fitting shoe, blocked arteries, and out-of-range blood sugar all at once. If each provider is seen individually, none gets the full picture. When all three providers share notes, decisions happen faster: imaging is ordered earlier, antibiotics get started sooner, revascularization happens before tissue dies, and offloading actually fits the patient's life.

You can read more about why coordination matters in this diabetes complications resource from the CDC and on our overview of diabetes and foot health. Whether it’s as simple as a house of cards or as complicated as a skyscraper, the right supports need to be in place to absorb and properly direct all the forces within. If something is off, problems arise. Now think of yourself as a building that moves. The forces against your body are constantly shifting—as you walk, run, stand, sit—and most of the time, your body is well built to keep everything on the level. But what happens when something is abnormal in your foot structure?

Turn to Sunshein Podiatry. Since 1991, we've guided patients in Greater Dayton through the blueprint of their feet and helped them reestablish a firm foundation of proper support and confidence. Here's what you need to know about abnormal foot structures and available treatment.

Table of Contents

What Happens When Your Arches Are Too High or Low?

High Arches

Low Arches

How Do These Arch Differences Develop?

How Do Our Centerville Podiatrists Help You With Abnormal Foot Structures?

What Happens When Your Arches Are Too High or Low?

Our feet are remarkably engineered to support our body weight and keep us moving forward. At the center of this design are the arches—structures that play a critical role in how we stand, walk, and adapt to the world beneath us.

Running from the heel to the base of the toes, the arches help absorb impact when our feet strike the ground and distribute pressure evenly as we move. They also provide balance and stability, especially when we’re walking on uneven surfaces. When arches function as they should, they protect not only the feet, but also the ankles, knees, hips, and lower back.

However, arches don’t always fall into an ideal range. When they’re too high or too low, the way forces travel through the body can change—often leading to discomfort, fatigue, or injury over time.

High Arches

Also known as cavus foot, this condition limits the foot’s natural ability to absorb shock. A healthy arch slightly flattens with each step, helping cushion impact. With high arches, the foot tends to remain rigid, placing extra stress on certain areas—particularly the heel and the ball of the foot.

People with high arches often experience pain while standing or walking and are more prone to issues such as hammertoes, claw toes, ankle instability, and thick calluses caused by uneven pressure distribution.

Low Arches

Low arches—commonly called flat feet—occur when the arch collapses or appears nearly flat against the ground. While flat feet often absorb shock well, the foot may roll inward excessively during movement, a pattern known as overpronation.

Overpronation can disrupt alignment throughout the lower body, leading to pain in the arch, heel, ankles, knees, or even hips. Over time, this altered motion can increase strain on muscles and joints that weren’t designed to handle the extra workload.

How Do These Arch Differences Develop?

In many cases, arch structure is inherited. If high or low arches run in your family, prevention may not be possible. In other situations, arch shape can change over time. The arch relies on a complex system of bones, muscles, ligaments, and tendons. Weakness, injury, or strain within this system can lead to gradual changes. Aging, excess weight, arthritis, diabetes, nerve conditions, or past injuries can all contribute to fallen arches or altered foot mechanics.

Children are also commonly born without visible arches, which typically develop as they grow. While this is usually normal, monitoring foot development over time is important to ensure arches form properly and complications don’t arise.

How Do Our Centerville Podiatrists Help You With Abnormal Foot Structures?

First, some good news: a structural abnormality doesn't always mean there's specifically a problem. If you have flat feet or high arches, but don’t feel any pain or other undesirable effects from them, then treatment isn’t necessary. It’s a bit like the Leaning Tower of Pisa. It’s not a normal shape, but it’s stable. Attempts to “fix” it only made it worse, so they’re not going to try anything drastic again until it’s needed!

However, if pain or other problems are a factor, then steps should be taken to manage these symptoms. Since an imbalance of forces is typically an underlying cause of problems, the corrective abilities of custom orthotics are a frequent consideration for treatment. At Sunshein Podiatry, we design medical orthotics to specifically address your needs, providing the support and balance that can relieve pain not only in the feet but also in other areas, such as the hips and knees.

When an abnormal foot structure contributes to other soft tissue injuries, such as heel pain caused by plantar fasciitis, our advanced MLS Laser therapy for pain can be an effective component of a plan for accelerated healing and relief.

As with any infrastructure, the sooner a problem is identified and addressed, the easier it is to avoid more severe problems down the road. Just look at our roads and bridges for proof! If you have been suffering from arch pain, heel pain, or ankle pain that just hasn’t improved, it’s time to get to the root of the problem. Our Centerville office is always happy to schedule an appointment with new patients.

5 Things to Keep Kids Busy (And Off Their Feet!) During Injury Recovery

Sometimes, foot and ankle issues develop in kids. There are many potential injuries and conditions, and each young patient presents a unique case. Now, children are usually quite energetic, and it can be difficult to try and keep them from expending their seemingly boundless energy by moving around when they need to sit still. When your child needs to spend time resting while their injury is healing, it can be helpful to keep them occupied. So, our Centerville foot doctors at Sunshein Podiatry offer these 5 suggestions to help keep your kid busy (and off their feet!) after an injury.

Table of Contents

Tip #1: Pick Up Some Books

Tip #2: Encourage Self-Expression Through Artistic Endeavors

Tip #3: Give Your Child’s Left Brain a Workout After a Foot Injury

Tip #5: Let Them Play (Some!) Parents benefit from being able to recommend both timeless classics and modern works from children’s and young adult (YA) genres. With so many options, you can definitely find books your child will be excited to actually read—even if they think they don’t like reading.

A particularly nice element to this activity is the fact that it doesn’t actually have to cost anything. Sure, you can purchase books—and if you do, consider a locally-owned or secondhand book store as possibilities—but you can also pick some up for free from the Washington-Centerville Public Libraries.

Even better, you can use this as an opportunity to spend some quality time with your child by forming your own little book club with just the two of you. Read the same book, come up with some questions, and have discussions about it! Your child will love the attention, and you can feel good about the fact you’re helping them develop reading and critical thinking skills.

Check out these great reading lists to expand your and your little one’s horizons:

Newbury Medal Winners and Honor Books. This award is “to the author of the most distinguished contribution to American literature for children” and is a surefire way to find awesome books!

The Ohioana Book Awards. State pride shines in this list that honors books that have been “written or edited by a person born in Ohio or who has lived in Ohio for at least five years.”

Children’s Book Council. Check out these diverse children’s and YA books that will help your kid expand their worldview!

Tip #2: Encourage Self-Expression Through Artistic Endeavors

One of the great things about the arts is that there are many options—any of which might hold particular appeal for your kid on foot rest. While drawing and painting are artistic endeavors that probably spring right to mind first, or perhaps even sculpting, the list doesn’t end there. Crafts, music, and writing are also potential interests, depending on what your child enjoys.

Regardless of which avenue for self-expression your child chooses, you're allowing them to explore their creativity. As with the previous option, you can also use this as an opportunity for quality time. No matter your particular artistic talents—or lack thereof—you can start a project with your kid. And if your abilities are, at best, not good, a little self-deprecating humor about the situation can make them laugh and hopefully understand the importance of being able to laugh at ourselves.

Here are some of our favorite resources for creative ideas:

Origami. A fun paper craft that jumps!

No-Sew a Blanket. Many different charities cherish handmade blankets like this – or get a start on gifts for Christmas!

Knit for Charity. Our favorite non-profit, Shoes 4 the Shoeless, distributes hand-knit scarves and hats during the winter to children in our community who need shoes. This would be a great local organization that would appreciate hand-knit scarves!

Tip #3: Give Your Child’s Left Brain a Workout After a Foot Injury

Crossword puzzles, sudoku, word searches, logic games, and other such activities require children to use the analytical and logical skills commonly associated with the left brain. When you do this, make sure to pick ones that are appropriate for your child. Both “too hard” and “too easy” can be problematic in different ways, so choose puzzles that provide just enough of a challenge to keep interest, without being so difficult that they get frustrated and give up.

You might also want to see if your child would be interested in attempting to piece together a traditional jigsaw puzzle. This is a great way to develop visual and spatial processing skills, while also making the process enjoyable and providing a sense of accomplishment. You may even want to offer to glue the completed puzzle together and frame it for their bedroom, and then grab another one for them to solve!

Need some resources? Try these:

Learning Express Toys. Visit this store in Dayton for story time, tot art programs, and more!

Print Your Own Worksheets. This site lets you choose the type of puzzle and even the general age range!

Get creative. Grab some graph paper and let your child create their own wordsearch, crossword, and more!

Build a model. Cars, plains, ships, and more!

Legos. Little kids and big kids alike love a good Lego build!

Tip #5: Let Them Play (Some!) Video Games

We fully understand if you have any initial resistance to this idea—there are instances where kids play video games way too much—but hear us out on this one. Yes, there’s no denying the fact that excessive video game playing can be a problem. However, the key to that is the word “excessive.” The way to avoid that is by establishing concrete time limits with your child. Naturally, the odds are pretty decent they might try to push the limits—“Just five more minutes!”—but stick to your guns!

One way to add value to the video game experience is to find ones that you can play with your child. As with the aforementioned suggestions to help keep kids busy after a foot injury, this is an opportunity to spend time with them and possibly even show that you’re willing to laugh at yourself. (Let’s be honest—your child is likely going to be way better than you at video games, so you might as well have a good time with it!)

Here are some kid-safe options you both might enjoy:

PBS Kids Games

10 Educational Video Games Your Kids Will Love

Educational Video Games

Tip #5: Homework (Yes, Really!)

No, homework isn’t a favorite for most children. But it's absolutely necessary. As a consideration in this matter, you may want to think about possibly using any of the previous options as an incentive for completing scholastic assignments. (“Once you’ve finished your math homework, we can play the video game for 30 minutes,” etc.)

Check out these free websites to help your child explore the world without even leaving the house!

GeoGuessr. Some rehabilitation will likely follow, and then probably a gradual return to your previous activity levels. It won’t pass as quickly as you want it to. It won’t always be easy. But it doesn’t have to be miserable!

In fact, there are many things you can do throughout your recovery to not only make the experience easier and more pleasant, but also to accelerate the healing process and achieve the quickest possible recovery. Our experienced Centerville foot doctors at Sunshein Podiatry understand what you're going through, and offer our top five things to do when you're recovering from foot or ankle surgery to stay healthy, stay sane, and plan for a better, more active future!

Table of Contents

Tip #1: Stock Up and Get Ahead

Tip #2: Coordinate With Your Caregivers

Tip #3: Prepare Your Home to Be a Place of Healing

Tip #4: Follow Your Post-Surgical Instructions Closely

Tip #5: Keep Your Brain Engaged

Put Your Trust in Us

Tip #1: Stock Up and Get Ahead

It's a good idea to make sure you’re all set for at least a couple of weeks on common household consumables, including paper products, garbage bags, pantry foods, and so on. If you have time to make some meals ahead of time and freeze them, that saves you not only a trip to the store but also a lot of time and stress in the kitchen.

Plan to complete any semi-regular chores before the procedure so you’re not scrambling for a clean shirt or a fresh towel a few days after surgery. Tasks such as laundry, cleaning, grocery shopping, taking out the trash, or even preparing meals can become surprisingly difficult when mobility is limited. You likely won’t be able to bear any weight—and we mean this seriously—for quite some time, making everyday responsibilities far more challenging without preparation or help.

Tip #2: Coordinate With Your Caregivers

For some people, this will be no problem at all. For others? Not so much. Not every person has an easy time asking for help. When you’re recovering from foot surgery, though, it’s important to make sure you have people around to support you, especially during those first few days and weeks after your surgery.

In addition to having someone on hand to drive you home immediately after the procedure, it’s good to make sure you’ll at least have a family member, friend, or neighbor drop by on a daily basis, or just be on hand to shop for you, clean for you, or help you with other tasks you can’t accomplish on your own.

Tip #3: Prepare Your Home to Be a Place of Healing

In the days and weeks after surgery, you’ll prefer to avoid any preventable accidents when moving around the house or up and down stairs. To help make your home easier to navigate, consider:

Adjusting your primary sleeping, dressing, and bathing areas to a single level or concentrated area.

Thoroughly clean your home of clutter, unnecessary furniture, cords, or other obstacles that could pose tripping hazards.

Setting up more night lights around the house so if you do have to move about in the dark, you can always see where you’re going.

Relocating any frequently used items, such as pantry items, cookware, clothing, and so on, to places where you won’t have to reach up high or bend over to access them.

Installing grab bars, tub chairs, non-skid mats, and other aids in bathrooms, kitchens, and other areas where extra stability is needed.

Tip #4: Follow Your Post-Surgical Instructions Closely

We can't overstate this: being mindful of post-operative care is often just as important as the surgery itself in terms of both the speed and quality of your recovery. Hopefully, this is your first—and last—surgery. This is all new for you.

However, our ankle and foot surgeons at Sunshein Podiatry have conducted thousands of them since 1991. So trust us when we say that we’re not just making this stuff up. Our guidelines are meant to protect you and help you heal as fully, safely, and quickly as possible. So even though you may be tempted to “cheat”—especially if things seem to be going well—make sure you stick to the treatment and post-surgical protocol outlined for your recovery.

Some general considerations and components we may include in your plan include:

Rest. Surgery recovery requires a lot of effort from your body. If it weren’t, we wouldn’t take as many measures as possible to avoid using it as a treatment option. Since it is, though, you will need to give your body the chance to perform its natural healing processes after the procedure.

Medication. Depending on your procedure and situation, we'll likely recommend or prescribe some form of medication for you. The pain-relieving properties of medicine certainly play a role, but the anti-inflammatory ones can be immensely helpful in assisting with your recovery.

Hygienic practices. The potential for infection is a significant risk associated with surgery. This risk doesn’t end once the procedure is completed, though. It's essential to keep any vulnerable areas clean to reduce the chance of developing an infection in recovery.

Restricted movement. Over time, your amount and range of movement will increase, but we may advise limiting the movement of the affected area for at least a certain period.

Assistive devices. We may issue or prescribe braces, casts, or other devices to help you keep weight off the repaired limb while still allowing you to be mobile.

Physical therapy. As you recover, it'll be necessary to gradually ease into physical movement. To that end, physical therapy plays a key role in post-surgical care. Stretching and strengthening exercises are essential for making sure your movement is as natural as possible.

Follow-up appointments. Don’t worry, you’re not on your own after the surgery! Our team schedules regular post-operative evaluations to monitor your progress and ensure everything is mending as it should.

We might also suggest MLS laser therapy to help your recovery, as it can help lessen pain, reduce swelling, elimiante the need for medications, and even help the surgical site heal more quickly.

Tip #5: Keep Your Brain Engaged

Boredom is one of the most challenging aspects for many people to overcome when recovering from foot or ankle surgery. This is especially true if you’re used to being extremely active in your daily life. So think about what you might like to focus on for the few weeks when you’ll need to rest your feet. Here are some ideas many of our patients have shared over the years:

Pick out some shows and movies to watch. Get up to speed on the latest dramas all the kids are talking about these days, or take some time to re-watch or catch up on the classics.

Select some new books. If you take a trip to the library a couple of days before surgery, you can arm yourself with hours upon hours of enjoyment as you recover.

Try different games or puzzles. Whether you prefer jigsaw puzzles, crosswords, sudoku, or massively multiple online role-playing games, now is the time!

Develop a new skill or hobby. Did you always want to learn to sew or play guitar? Make more time for your scrapbooking? Learn how to code? While you’re waiting for your foot to heal, what’s stopping you?

Write some letters. A hand-written card or letter to someone you haven’t connected with in a while can be a meaningful experience—for you and the person you’re writing to!

As your foot continues to heal, we may clear you to begin reintroducing some non-impact or low-impact exercises to help you get moving again.

Put Your Trust in Us

Our podiatrists fully understand that nobody enjoys dealing with the aftermath of a foot or ankle surgery. They appear on the soles of your feet, may cause discomfort while walking or standing, and are challenging to remove, especially with over-the-counter remedies. However, with the right treatment approach, you can effectively alleviate symptoms and eliminate the human papillomavirus (HPV) that causes plantar warts. But how?

At Sunshein Podiatry, we rely on Swift microwave therapy, an effective and efficient solution for removing troublesome plantar warts. Let's take a moment to review what they are, how they develop, and why microwave treatment zaps them for good.

Table of Contents

Why Do Plantar Warts Develop?

Plantar Warts Symptoms

How Does Swift Therapy at Sunshein Podiatry Help Eliminate Plantar Warts?

Is Swift Therapy Right for You?

Why Do Plantar Warts Develop?

These are non-cancerous skin growths caused by certain strains of HPV that appear on weight-bearing areas, such as your forefoot or heel. HPV virus enters the body through tiny cuts or breaks in the skin on the feet. It thrives in warm, moist environments such as swimming pools, locker rooms, and communal showers, making these places common sources of infection. The virus spreads through direct contact with contaminated surfaces or by walking barefoot in these areas. Individuals with weakened immune systems or those who have had prior exposure to it are more susceptible to developing plantar warts.

Plantar Warts Symptoms

Identifying the warning signs early is crucial for prompt treatment. Here are some common signs to look out for:

Small, grainy growths on the soles of the feet.

Rough texture with a cauliflower-like appearance.

Pain or tenderness when walking or standing.

Discomfort when pressure is applied to the wart.

Black dots, also known as wart seeds, on the surface of the wart.

Callus formation around the wart due to increased pressure.

How Does Swift Therapy at Sunshein Podiatry Help Eliminate Plantar Warts?

Swift therapy is an innovative and advanced treatment method specifically designed to target and eliminate plantar warts. It utilizes microwave technology to deliver precise and controlled energy to the affected area, effectively destroying the wart tissue while minimizing discomfort and promoting faster healing. One of the key advantages of this approach is its ability to treat warts quickly and efficiently. Unlike traditional treatments that may require multiple sessions, microwave treatment often achieves complete wart removal in just a few short sessions, typically spaced several weeks apart.

Additionally, this therapy not only targets visible warts but also addresses the underlying HPV infection responsible for their development. Zapping the virus at its source helps reduce the risk of recurrence and offers long-lasting results.

During the procedure, a specialized handheld device is used to administer low-level microwave energy to the wart. This energy heats the tissue, stimulating an immune response and triggering the body’s natural healing processes. Over time, the body recognizes the wart as a foreign invader and actively works to eliminate it. Swift therapy is generally safe and can be performed on an outpatient basis. It may cause mild pain or discomfort during the procedure, but that is about it—you can return to your daily activities after a session.

Is Swift Therapy Right for You?

Our experienced Dayton-area foot doctors conduct a comprehensive evaluation before making any treatment recommendations. We're eager to understand your "why" and identify the root cause, not just the symptoms. Here are some considerations we'll discuss with you:

Type and severity of warts. If you have other kinds of warts or growths in different locations, Swift therapy may not be the most suitable option.

Previous treatments. If you've tried other remedies without success or experience recurrent plantar warts, microwave treatment may be a viable alternative worth considering.

Health status. Your overall health and medical history play a crucial role in determining the most suitable care protocol. It’s important to inform our team about any existing medical conditions, medications you're taking, and any allergies or sensitivities you may have.

Treatment preferences. Some individuals may prefer non-invasive treatments that offer minimal discomfort and quick recovery. If you value these aspects, Swift therapy aligns with these preferences.

To make an informed decision, consult with our skilled medical team to develop a personalized approach that helps you enjoy long-term relief from plantar warts and return to your active lifestyle.

Laser Treatment: A Powerful Tool for Sports Injury Healing

Athletics are a healthy way to stay fit, but they also carry the risk of foot and ankle injuries that cause immense pain and impact performance. Laser energy photons penetrate the skin around the injured region, where the body absorbs them and facilitates cell regeneration and growth. Most importantly, laser therapy stimulates blood flow to the injured area, which delivers oxygen, water, and nutrients to accelerate healing.

The use of light in therapy has been around for decades since a physicist invented the laser in 1958. Later in the 1960s, Endre Mester discovered the medical use of laser therapy after noting that when he shaved the back of a rat and illuminated light on it, the hair grew faster.

Since then, laser therapy has gained popularity and received FDA approval. Doctors now utilize this technique to treat a wide range of injuries and diseases. Additionally, using light energy helps minimize discomfort by stimulating the production of endorphins, which aid in pain management.

Why Is Laser Therapy Useful for Treating Sports Injuries?

Sport-related foot and ankle issues are mainly caused by overuse of tendons, ligaments, and joints. In addition to reducing severe pain and inflammation, laser treatment accelerates new cell regeneration in the affected areas, promoting more effective healing. As a result, laser treatment also improves muscle performance during physical activities. When injuries take longer to recover, numbness and reduced mobility often result. However, this approach also helps improve nerve regeneration, which otherwise results in loss of motor function.

Many of our patient-athletes also appreciate that MLS laser therapy is non-invasive and has minimal side effects, making it an ideal treatment for sports injuries and helping them return to peak performance more quickly.

What Are the Main Sports Injuries That Benefit From Laser Treatment?

Laser therapy enters the skin and effectively treats sports injuries that most doctors only treat through surgery and injections. While light from a laser can be intense, our skilled Centerville podiatrists control the beam to focus only on destroying damaged tissues. Many of our patients experience great success using laser treatment for the following conditions.

Achilles Tendinitis

Your Achilles tendon is on the back of the ankle and is often injured during strenuous physical activity or overuse. Laser therapy helps relieve Achilles tendinitis symptoms, such as pain and swelling, by increasing blood flow to the tendons and enhancing healing. Additionally, this treatment also minimizes nerve sensitivity and reduces pain.

Plantar Fasciitis

Athletes and sports lovers constantly exert pressure on their feet, which can lead to plantar fasciitis. The plantar fascia tissue then weakens and causes a stabbing pain in the heel. Laser therapy accelerates tissue repair by penetrating deeply into the heel, thereby reducing inflammation. The treatment also stimulates the immune system to fight inflammation and reduce pain.

Ankle Sprains

It’s common for anyone to twist, turn, or roll their ankle, resulting in a sprain. When this happens, the strong ligaments supporting the ankle tear and cause pain. Repeated sprains result in bigger issues like arthritis, thus the need to treat them with laser therapy. The treatment stimulates tissue growth and regeneration, reduces heel inflammation, enhances blood flow, and promotes better healing.

Does Laser Therapy Speed Up Recovery Time?

Yes, this method is the way to go if you want a faster recovery. Unlike more traditional treatment options, which require a lot of rest and therapy during recovery, MLS laser therapy expedites healing by repairing and generating new cells and providing nutrients and oxygen to the affected area. Within one to two treatment sessions, you'll notice a reduction in pain and inflammation and, ultimately, a quick recovery.

Are you experiencing excruciating pain after a sports injury, or are you struggling with recurring conditions that take a long time to heal? MLS laser therapy at Sunshein Podiatry is the key to ending the pain and reclaiming your sports freedom, whether it’s a new or old injury.

Why Cracked Heels Keep Coming Back—and How to Stop Them

The thick skin on your heels plays a crucial role in the overall health of your feet. It provides vital cushioning to absorb the shocks associated with weight-bearing movements, such as walking, running, and jumping. However, if the skin on your heel starts to crack, flake, and peel away, it’s important to address this problem as soon as possible. While cracked heels aren't a major cause for concern, severe cases can lead to infection or more serious foot issues, such as altering your gait to avoid pain while walking.

Schedule a consultation with Sunshein Podiatry. As pressure from standing or walking increases, this hardened skin can split, forming painful cracks that deepen over time. In more severe cases, these fissures can bleed, making everyday activities uncomfortable and causing frustration or self-consciousness about the appearance of your feet.

Dry skin is the primary contributor to cracked heels, though certain individuals are more prone than others:

Older adults often experience heel cracking due to a natural decline in skin oil production.

Athletes such as runners and swimmers place repeated stress on the heels that can worsen dryness and irritation.

People managing diabetes can also develop heel fissures if they're not able to attend to daily foot care and moisturizing.

Environmental and lifestyle factors also play a role, including cold weather, prolonged standing, poorly fitting shoes, and open-back footwear such as sandals or flip-flops. Long, hot showers and harsh soaps can further strip moisture from the skin, increasing the risk of cracks.

The encouraging news is that cracked heels are both preventable and treatable. With proper care and targeted remedies, dry, damaged skin can be restored to a smoother, healthier state.

What Can Be Done to Heal Cracked Heels?

Cracked heels may seem like a minor nuisance at first, but when dryness deepens into painful fissures, they interfere with your comfort, mobility, and confidence. But with some simple home remedies, you can treat cracked heels—and maybe even prevent the problem from happening in the first place. Below, we offer a few tips to restore moisture and protect your feet.

Wear Socks

Socks help regulate moisture and keep your feet comfortable by drawing sweat away from the skin. Proper moisture control plays an important role in preventing cracked heels. Socks also act as a protective layer between your heels and your shoes, minimizing friction and reducing the risk of further irritation.

Use Moisturizers

Regularly moisturizing your heels helps keep the skin soft, flexible, and better able to withstand daily pressure from walking and standing. Hydrated skin is less likely to thicken, crack, or split, reducing discomfort and the risk of painful fissures. Consistent moisturizing also supports faster healing, improves the appearance of the skin, and helps prevent heel fissures from returning.

Castor oil and petroleum jelly are heavy and creamy, making them ideal for cracked heels. We also recommend medicated lotions at The Foot Stop Store, located in our Centerville practice.

Pumice Away Dead Skin

A type of natural volcanic rock, pumice stones exfoliate your cracked heels and soften the skin. This practice makes it easier for moisturizer to quickly penetrate the skin and properly hydrate those hurting heels. However, keep in mind that when using a pumice stone, rub the area gently and avoid over-exfoliating.

Wear Proper Shoes

Some footwear choices—sandals, for example—aren't recommended if you have cracked heels. Instead, shop for supportive sneakers that provide the ample cushioning your heels need, which helps prevent fissures from deepening.

Drink Plenty of Water

Staying well-hydrated helps heal cracked heels from the inside out. When your body has enough water, your skin is better able to maintain elasticity and retain natural moisture, making it less prone to dryness and splitting. Proper hydration supports overall skin health, complements topical treatments, and helps create the conditions needed for cracked heels to heal more effectively.

If Simple Remedies Aren't Enough For Your Cracked Heels, Get Help From Sunshein Podiatry

If heels fissures persist despite home care or become painful, deep, or prone to bleeding, professional treatment has a meaningful impact. Our foot care specialists can safely remove thickened skin, address underlying causes, and recommend targeted treatments that promote faster healing and prevent recurrence. Seeking expert care not only relieves discomfort sooner but also helps protect your feet from complications—so you can walk comfortably and confidently again.

Find Heel Pain Relief With Custom Orthotics

Heel pain is far more common than many people realize. In fact, nearly one in four Americans will experience a serious episode of it at some point in their lives—often severe enough to interfere with daily activities and overall quality of life. If you’re dealing with ongoing discomfort, you may find that even simple activities such as walking, exercising, or enjoying your favorite hobbies have become difficult or frustrating.

While heel pain can stem from many factors—such as weight changes, unsupportive footwear, or repetitive stress—one of the biggest contributors is the modern environment. Our feet simply weren’t designed to withstand hours spent on hard, unforgiving surfaces like concrete and asphalt. Over time, this constant impact can strain the foot's structures and lead to conditions such as plantar fasciitis or Achilles tendinitis. The good news is that effective treatment is available at Sunshein Podiatry. Our experienced team of doctors designs custom orthotics to relieve heel pain, reduce stress, and improve foot alignment.

Table of Contents

What Causes Heel Pain?

How Do Custom Orthotics Relieve Heel Pain?

What Kind of Orthotics Will Help Me?

What Causes Heel Pain?

When the muscles and tendons that support the arch weaken, it causes a cascade of problems. The arch drops. The ankle rolls inward. The foot and leg bones fall out of alignment with each other. Tender tissues become strained—or even tear. And the heel is right at the center of it all, forced to absorb ever-greater impact forces with every step you take. Some of the most common conditions related to chronic heel pain include:

Plantar fasciitis. Risk factors associated with this condition include being overweight, working or exercising on hard surfaces, and wearing ill-fitting shoes.

Achilles tendinitis. When the Achilles tendon swells after an injury, it causes joint pain and stiffness. Ankle braces and custom orthotics help reduce pain and swelling during recovery.

Heel fracture or ruptured tendon. These foot and ankle injuries cause sharp heel pain, swelling, and mobility issues. In addition to custom orthotics, our podiatrists may recommend further treatments to help alleviate heel pain and improve mobility.

Bursitis. Often characterized by a dull ache, this condition occurs when foot joints become painful and swollen. Other symptoms include redness, tenderness, and warmth in the affected area.

How Do Custom Orthotics Relieve Heel Pain?

Orthotics are shoe inserts designed to support the feet and relieve pain associated with various podiatric issues, from arthritis to foot deformities. Now, you've probably seen generic inserts in the drug store, and maybe even tried them. While off-the-shelf options are relatively cheap, they often fail to provide much relief, are likely to wear out quickly, and, most importantly, aren't designed to specifically address the needs of your foot mechanics.

At Sunshein Podiatry, our doctors recommend custom orthotics designed to control foot alignment and provide adequate cushioning. Some of our medical devices are made using soft compression materials and are very comfortable to wear. Others are designed using rigid materials, such as plastic or carbon fiber, which help support your arches. Specially fitted orthotics help reduce pain, improve mobility, enhance posture and balance, reduce the likelihood of falls, and prevent the need for more invasive treatments, such as surgery.

A helpful analogy here is to think of orthotics as a pair of glasses for your feet and arches. If you have a foot that's imbalanced or unstable, custom orthotic insoles can reposition and support it so that it functions as intended. However, glasses won't improve the quality of your biological eyes, but the right pair of prescribed orthotics helps the muscles in the bottom of your foot heal and become stronger, allowing them to better resist gravity and reduce pain. In fact, many of our patients wear orthotics as a preventative, long-term measure to tackle heel pain.

What Kind of Orthotics Will Help Me?

Deciding on the right type of orthotics to relieve your heel pain starts with a comprehensive examination in our Centerville office. Our experienced podiatrists carefully evaluate your condition, learn about your "why" to better understand you—after all, a treatment plan that works for a marathon runner won't work for a busy teacher who stands all day. Then, we consider what type of medical orthotics best serve your needs. Here are some options:

Prefabricated arch supports. For our patients with mild-to-moderate heel pain, we carry Powersteps arch supports, available at The Foot Stop Store in our office, and they tend to work well for most people. They’re a lot higher quality than off-the-shelf inserts you find at the pharmacy or athletic store, and you’ll have a professional foot and ankle specialist helping ensure you get the proper pair.

Custom orthotics. If your heel pain and/or foot alignment problems are more severe, you may need a pair of custom-molded orthotics. Each pair is uniquely fashioned in a lab, using a mold and/or scan of your feet. This allows our podiatrists to correct foot alignment with a much higher degree of precision, especially for foot structures that deviate a lot further from “normal.”

Custom orthotics also have the advantage of being significantly more rigid and durable than prefabricated ones. Since they’re designed to fit perfectly, they can be made from firmer plastics while providing enough “flex” to be comfortable and allow your foot to function normally. And they last for many years with proper care, unlike prefabs—which tend to give out after about six months or so.

Ultimately, we want to give you the opportunity to not only learn about what's causing your heel pain, but also learn the various ways Sunshein Podiatry can help you resolve it and return to your active lifestyle.

Put Your Best Foot Forward and Alleviate Foot Pain With Custom Orthotics

Chronic foot pain is one of the most common obstacles to achieving your personal activity goals, and also one of the most overlooked. But fortunately, it’s something that can usually be treated successfully without surgery—and quite often, custom orthotics are part of the solution.

Whether you’ve never had custom orthotics before and are struggling with foot pain, or you already have a pair you wear regularly and love, now may be the perfect time to pick up a new pair. Work with the foot and ankle specialists at Sunshein Podiatry. Since 1991, we've helped people throughout Greater Dayton benefit from the effective, non-invasive relief that orthotic devices provide.

Table of Contents

Why Are Custom Orthotics So Versatile and Effective for Foot Pain?

Why Might Two or More Pairs Be Better Than One?

Make a Valuable Investment in Foot Health and Comfort

Why Are Custom Orthotics So Versatile and Effective for Foot Pain?

First, an important caveat: orthotics aren't going to be the solution to every foot problem. Not everyone will require them. Some people do fine with over-the-counter, prefabricated arch supports. Others may not need arch supports at all. However, there’s a reason medical orthotics are frequently a great solution for many types of foot, leg, and even hip and lower back pain, and it has to do with biomechanics and alignment.

A significant percentage of painful foot problems—including many forms of heel pain, overuse injuries, and even foot deformities like bunions—are partially the result of feet that have fallen out of alignment with the leg. Weak or flat arches, overpronation, and other structural or biomechanical foot problems can increase the amount of pressure and stress on certain areas of the foot, and also force joints and muscles further up the body to work harder in order to compensate.

Wearing the correct set of orthotics for your foot shape and condition—not the flimsy drugstore generic variety—helps restore proper alignment, relieving strain and pain throughout the feet and beyond. And prescribed insoles are going to give you the best possible performance, since our foot and ankle specialists design them to fit you and you alone, using precise molds of your feet.

They also last much longer than off-the-shelf orthotics—typically 3–5 years on average, as opposed to 6 months or less for cheap retail alternatives.

Why Might Two or More Pairs Be Better Than One?

But maybe you already knew all of that—because you already have a pair of orthotics, and you feel the benefits every day! If so, we're so happy they’re working out for you! Here’s a question to ponder, though: have you ever considered picking up an additional pair? We know, we just said custom orthotics last up to 5 years. That’s a pretty decent lifespan already. Why would you need to pick up a second set so soon?

There are actually a number of good reasons:

They’ll last even longer. When you have one pair of orthotics, they’re holding up your entire weight all day, every day. When you have two or more pairs, now the load is being shared. That 3–5 year expected lifespan for a single pair could easily become 6–10 years for two, condition permitting, if you cut the wear and tear in half for each set!

We can customize them for specific footwear. Our podiatrists want to know your "why" so we can better serve your lifestyle. No two pairs of shoes are exactly the same. Dress shoes, for example, may run a little narrower and have less give and wiggle room than casual athletic shoes. If you have only one pair of orthotics, they might fit really well in one pair of shoes, but not as well in another. However, using your footwear as a guide, we typically modify your medical inserts at our Centerville office so you can have one pair for one type of shoe, one pair for another, and a great fit for both.

They’re less hassle. We typically recommend our patients wear their orthotics every day, all the time. If you have several different pairs of shoes and boots that you wear regularly, that means you’re switching your orthotics constantly between them. More choices for insoles mean fewer swaps, greater convenience, and make it more likely that you can just throw on your shoes and go!

There’s one more thing to consider, and that is—of course—finances. Many insurance plans will fully or partially cover new orthotics at regular intervals, often yearly, even if an older pair is still in good shape. You may also be approaching a deadline to spend tax-advantaged FSA money. If you're in either of these situations, acquiring a new pair of devices now actually makes financial sense, too.

When you consider all the benefits, the reality is that for most patients, adding a second pair of orthotics to their collection provides more than double the value of a single pair.

Make a Valuable Investment in Foot Health and Comfort

Don’t let foot pain keep you from all the things you love. The team at Sunshein Podiatry can help you restore comfort and mobility and get that spring back in your step. Custom orthotics—whether they’re your first pair, or just a new pair—may well be the answer. Or, they may not be. Either way, our doctors are trained to help you find the best path forward to total foot and ankle relief.

Stop Nerve Pain in Your Feet Before It Stops You

When major problems develop suddenly and dramatically, like most people you probably make fixing them a very high priority. Broken ankle? Pipe burst in the basement? There are problems that are impossible to ignore. But what about problems that emerge slowly over time, over the course of months or even years? You might not even notice there’s an issue at first. And it never seems to be all that much worse than it was yesterday.

Peripheral neuropathy is definitely this kind of problem. During the lengthy, early stages of the disease, you might find that it doesn’t really affect your day-to-day life all that much. But in time, little by little, it can rob you of your ability to play with your kids and grandkids, or drive your car, or remain independent. It could even lead to more complicated injuries and risky infections. So don’t wait! Take those intermittent tingles and aches seriously, and contact Sunshein Podiatry. We serve the entire Dayton area with exceptional foot and ankle care. Let us help you stop nerve pain with comprehensive neuropathy treatment.

Table of Contents

What Are the Stages of Peripheral Neuropathy?

Who's at Risk for Developing Peripheral Nerve Damage?

The Earlier You Seek Help, the Better Your Outcome

What Are the Stages of Peripheral Neuropathy?

Let’s take you through the stages of what this disease can do to your feet and your body. Think of it as a curve where symptoms start to increase and then decrease—but not in a good way.

When the peripheral nerves in your feet and toes first start to become damaged, you may not notice anything at all for a while. Eventually, though, you may occasionally feel intermittent episodes of mild to moderate pain, tingling, or other unusual or unexplained sensations. These episodes may be spaced weeks or even months apart.

As you start climbing the curve, the symptoms slowly start to become more noticeable—and more frequent. By the time you get to the absolute peak of the curve, your pain is constant and probably so intense that you’re on a bunch of medications in an attempt to control it.

Then, as you start heading down the other side of the curve, pain becomes less and less severe. Unfortunately, this isn't because you’re getting better. It’s because your nerves are dying, and your feet are on the way to becoming totally numb.

The lack of feedback from these vital signals also has grave implications for your balance, your ability to walk and be active, your ability to drive, and your ability to notice injuries before they become severe. Sadly, we've heard far too many stories of people with severe neuropathy becoming socially isolated, unable to partake in activities that they used to enjoy. And they are much more likely to develop infected wounds that may force feet amputation.

Who's at Risk for Developing Peripheral Nerve Damage?

By far, the most commonly related condition to peripheral neuropathy is diabetes. When your blood sugar isn’t properly regulated, the associated inflammation and reduction in blood flow can be extremely damaging to the small, delicate nerves in the feet and ankles. For this reason, it’s believed that a majority of people with diabetes will develop a form of neuropathy, to certain extent, at some point in their lifetime. The longer you've lived with diabetes, the greater your risk.

However, diabetes isn't the only possible cause of nerve pain and damage. It’s also associated with a variety of other factors, including but not limited to:

Smoking

Alcohol misuse

Nutritional deficits

Several different systemic medical conditions

It’s also critical to understand that sometimes neuropathy has no known cause that can be identified. This is called idiopathic neuropathy, and up to 10% of neuropathy sufferers are in this category.

What this means is that you should always take the early warning signs of neuropathy seriously, even if you aren’t aware that you have any personal risk factors. Intermittent, unexplained tingling and nerve pain in the feet and ankles should never be ignored.

The Earlier You Seek Help, the Better Your Outcome

When a condition progresses as slowly as neuropathy often does, you aren’t just battling your disease, but also inertia. If symptoms aren’t bad yet, and may not be bad for a while yet, you might not feel a sense of urgency to act. Here’s the problem, though. By the time you start to notice even subtle neuropathy symptoms, there’s a chance your nerves have already taken pretty heavy damage. And unlike most other types of tissue, nerves aren’t always capable of fully repairing or regenerating themselves.

In addition to treatments from your podiatrist, successfully managing neuropathy requires making intentional, disciplined choices, including eating a healthy diet, exercising regularly, and developing other proactive lifestyle habits. And it will take time to get things under control. Exercise improves blood and nutrient flow to the legs and feet—and the nerves they contain— reduces swelling, and strengthens and stabilizes weakened muscles and supporting tissues. And of course, if your neuropathy is linked to diabetes or high blood sugar, regular exercise will help you keep those exacerbating factors in better control.

This means that exercise not only makes your feet and legs feel better but also keeps your neuropathy from advancing to a more severe stage. This reduces your risk of falls, wounds, and other severe complications.

However, on the other hand, certain forms of exercise can be dangerous for someone who has already lost a lot of feeling, strength, and/or balance in their lower limbs. There's an increased risk of falling or suffering other kinds of accidental injuries. There's also a lack of force feedback from your feet. This means you could suffer a cut, pull your Achilles tendon, or even break a bone without immediately feeling it. The longer a medical issue goes unnoticed, the greater your risk of developing severe infections or deformities.

Many of our patients at Sunshein Podiatry come to us with this dilemma, and we totally understand their frustration. So, our experienced podiatrists want to share these safe exercise tips for peripheral neuropathy if you're facing a similar challenge.

Table of Contents

What Are Some Low-Impact Exercises People With Neuropathy Can Do?

Easy Cardio

Strength and Balance Training

Range of Motion Exercises

How Can You Prepare Safely for Exercising?

What Are Some Low-Impact Exercises People With Neuropathy Can Do?

Before you lace up or suit up, we'd like to stress a couple of important points:

Stick to exercises that put less weight, pressure, and risk on your feet and ankles.

Focus on movement that specifically counteracts some of the most common problems associated with neuropathy.

Let’s break these categories down a bit.

Easy Cardio

Cardiovascular fitness exercises are a crucial component of overall health and well-being for all individuals. Regular cardio helps weight management, improves blood sugar control, provides more oxygen and nutrients to your lower extremities, and even promotes better sleep—all of which have profoundly positive effects on the course of your neuropathy.

The problem is many cardio exercises—such as running, pickleball, jump rope, basketball, or tennis—are high-impact. In other words, they place heavy demands on your feet, ankles, and joints. If your lower limbs have been weakened by neuropathy, these activities may be too severe to perform safely.

So here are alternative cardio exercises we recommend:

Walking. Although it may be lower in intensity than running, brisk walking is still an excellent cardio exercise—especially if you can manage at least 30 minutes at a time and have supportive footwear.

Cycling. Both going outside for a bike ride or riding a stationary cycle are great forms of cardio that put less stress on your feet. The stationary bike may be preferred if your neuropathy is severe, you're unsure of how far you can comfortably ride, or are uncertain about weather conditions.

Aquatics. The natural buoyancy properties of water allow you to get great cardio while putting almost no strain on your joints. Swimming laps is great, and so is water aerobics.

Strength and Balance Training

Stronger muscles provide better support and balance, which helps you stay more resistant to injuries. Plus, with regular conditioning, your muscles become more efficient at using the oxygen delivered by your blood.

Fortunately, many of these exercises can be performed at home with little to no specialized equipment. For instance:

Calf raises. Slowly rise up to your tiptoes, hold the position as long as you comfortably can, then slowly lower. For support, use a wall, kitchen counter, or sturdy piece of furniture. As you get more comfortable, try one foot at a time, with your other foot lifted behind you.

Heel-to-toe walking. A great one for balance! Simply walk in a straight line by placing your heel directly in front of the toes of the other foot with each step. The shoes should touch.

Seated dorsiflexion exercises. Sit in a comfy but straight-backed chair with your feet flat on the floor. Then, keeping your heels on the floor, flex your feet and toes upward as high as you can, hold, and slowly let them down.

Of course, if you do like to go to the gym, you can try the leg extension machine, hamstring curl machines, and other low-impact strength training exercises. However, avoid exercises that require heavy weight bearing while standing, such as barbell squats.

Range of Motion Exercises

Flexibility stretches are also a key part of building up resistance to peripheral neuropathy-related injuries. They're foundational to all the other forms of exercise you engage in, since they help warm up your muscles and joints and allow you to safely sustain higher levels of activity. Here are a couple of good examples:

Calf stretches. Face a wall—close enough to put your hands on it for support if needed—with your feet shoulder-width apart. Then, step back with one leg as far as you comfortably can, with your back foot flat on the floor and the back knee only slightly bent. You’ll have to bend your front knee. Lean forward as well as you can, then hold the stretch at least 10–15 seconds. Switch and repeat.

Ankle circles. This stretch is really straightforward. Sit in a chair, with one leg crossed over the other knee. Move your ankle in a slow, steady circle at least 10 times in each direction. Alternatively, you can try imagining your big toe as the tip of a pen and writing out the alphabet using only your ankle. If you can't cross your leg, hold up one leg under your thigh and rotate your ankle, then switch to the other leg.

How Can You Prepare Safely for Exercising?

We hope you’re excited to reintroduce healthy movement into your daily life. However, there are a couple of safety checks to keep in mind:

If you have noticeable signs of peripheral neuropathy, always seek the input of a physician—whether your personal care provider or one of the doctors here at Sunshein Podiatry—before putting together a comprehensive workout plan. We’ll help ensure your plan is well-suited to your unique situation and personal needs, from both a safety and optimal benefit perspective.

Start slowly at first, especially if you're not accustomed to regular exercise or are making significant changes to your previous routine. It will take your body time to adapt to the new kinds of stresses and challenges. Better to move at a pace or intensity you know you can handle, and then increase by no more than 10-15% per week.

Always make sure you have the appropriate safety gear, including the right kind of athletic shoes in good repair and fit your feet well. Once again, our team can assist you with this.

To help you with the at-home component, we’ve written a brief downloadable guide on health and wellness. Keep it on your phone or print it out to keep exercise guidelines easily accessible and top of mind!

No Need to Struggle With Knee, Hip, or Back Pain When Custom Orthotics Can Help

You might not regularly think of your body as a structure, but it is. A mobile, organic structure with a great many interconnected moving parts. When all the parts of our body are as they should be, these great machines of ours are able to go through the day with little effort or trouble. If you’re not thinking about how your body is moving, it likely means everything is functioning properly.

But when you're experiencing pain, every step you take starts to become thought out and calculated. You feel every move you make, because it triggers pain in your feet, legs, back, knees, or hips. And while your focus may be on those areas in pain, the root of the problem might mostly be on the bottom floor, so to speak. Our experienced Centerville foot doctors at Sunshein Podiatry know what can provide the support you need: custom orthotics. Let's take a closer look.

Table of Contents

Why Does Your Foot Foundation Influence the Rest of Your Body?

How Can Sunshein Podiatry Help Restore Your Structure With Custom Orthotics?

Why Does Your Foot Foundation Influence the Rest of Your Body?

Whenever you walk, run, or even stand, you're able to do so because your feet distribute the weight of your body in a stable manner. While a house doesn’t move, its foundation also supports its structure. But if that foundation becomes unstable or falls out of alignment, you know how this affects the home resting on it. Structural abnormalities have similar effects on your body. While you can certainly develop foot pain or heel pain from conditions such as flat feet or high arches, the effects can run up your musculoskeletal structure.

Let’s start at the bottom. Your feet are tasked with supporting and properly distributing your weight, and that can be quite a lot! For every pound you weigh, about four pounds of force need to be handled by your feet whenever you move. And that’s every step. So, a problem with your foot structure changes the distribution of weight across the foot, and leads to a higher concentration of stress in areas that aren’t built to handle it. This increases the risk of painful foot problems such as plantar fasciitis and Achilles tendinitis.

But that's not all. A deformity in your foot structure can change the way you walk, also known as your biomechanics. For some individuals, an abnormal foot structure causes an uneven gait. One common instance is overpronation, in which the foot rolls too far inward with each step.

But our feet are not independent pieces of our bodies. They’re connected to our ankles, which are connected to our legs, our knees, our hips, and all destinations upward. When our feet move in an abnormal way, it often causes other parts of the body to shift as well in order to compensate. In the case of overpronation, your legs might begin to rotate inward. Our legs and knees also tend to work overtime to keep us more stable while running. In most cases, you won’t even realize it’s happening, but all these changes place extra strain throughout the body, which contributes to fatigue and once again, leads to pain.

Just keep moving up the chain with abnormal stances and overcompensation, and it’s no surprise that you can end up with backaches, too!

How Can Sunshein Podiatry Help Restore Your Structure With Custom Orthotics?

When many of these structural problems can be traced back to the feet, addressing them there also helps align further up the musculoskeletal system. Depending on the conditions at play, we may recommend one of several different treatments. In many of these cases, custom orthotics will be a strong candidate. A pair of prescribed orthotic inserts—not the generic, off-the-shelf variety—provides the exact amount of cushioning and corrective support where your feet need it most. They can help alleviate excess stress in specific areas, as well as prevent overpronation and other abnormal gait patterns.

Setting the feet back to proper weight distribution and biomechanics will likely have positive effects in areas above as well. Stiffness, pain, and fatigue can significantly lessen or even vanish entirely!

Depending on your condition, and to complement the corrective nature of orthotics, our medical team might also add the following to your treatment plan:

Stretches or exercises to condition the supporting elements of your body. These not only help improve endurance, but also reduce the strain on tight muscles and tendons in other areas.

Changes in footwear, physical activity routines, and/or work environment to reduce stress and prevent overuse.

Advanced treatments such as MLS laser therapy to accelerate recovery of soft tissue injuries.

Our main goal is always to use conservative, non-invasive methods whenever they have the potential to be effective. No two patients are the same. The causes of pain for one may be different from those of another, so we can’t guarantee that back pain and foot pain are always connected.

However, what we can guarantee is that we'll always perform a thorough evaluation and consider all your needs—we call this "listening to your 'why'". This includes understanding your lifestyle, how symptoms affect your daily activities, and your wellness goals. Since 1991, thousands of patients in the Greater Dayton area have placed their trust in our team. A tree that has been felled and cut into lumber cannot become a living tree again. Scrambled eggs can never be uncooked. For a long time, doctors thought that nerve damage from peripheral neuropathy was irreversible, too—at least by using the treatments available at the time. But now, it’s time to take peripheral neuropathy off that list.

Thanks to cutting-edge research and advanced treatment options like Neurogenx, many people with neuropathy are not only reducing their reliance on painkillers, but also actually regaining nerve function that they thought they’d lost forever. At Sunshein Podiatry, we’re proud to be part of a small but rapidly growing group of medical experts who help patients reverse their neuropathy symptoms with Neurogenx and return to more active, fulfilling lifestyles.

Table of Contents

What Should You Know About Neuropathy?

How Is Neurogenx a Revolution in Neuropathy Treatment?

What Should You Know About Neuropathy?

Before we move on, let’s take a brief refresher course on this all-too-common condition. Peripheral neuropathy is an umbrella term for progressive damage to the sensitive nerves of your feet and toes. High blood sugar levels due to diabetes are the most common underlying cause, but certainly not the only one—injuries, autoimmune disorders, exposure to toxins or certain medications, alcohol use disorder, and other factors can play a role. Sometimes there's no known underlying cause.

Neuropathy usually progresses gradually, over the course of years or even decades. However, slowly but surely, as the nerves become more and more damaged, symptoms worsen. Intermittent tingling and burning sensations eventually evolve into constant, significant pain. Eventually, all sensation is completely lost, leaving your feet numb—and extremely susceptible to devastating injury.

How Is Neurogenx a Revolution in Neuropathy Treatment?

A caveat before we go any further: No, we’re not saying that this approach is going to make you feel like a kid again. Nor can we promise perfect results in every case. It’s not magic. But at the same time, “revolutionary” is absolutely a fair word to use if you want to:

Significantly reduce the amount and frequency of medication you take to control your neuropathy—or maybe even eliminate it altogether

Regain much of the feeling and nerve function you thought you’d maybe lost forever

Return to activities you used to enjoy

Our Neurogenx neuropathy treatment provides you with the best opportunity to achieve these goals. Clinical studies show that 4 out of 5 people who undergo this treatment experience a significant reduction in symptoms and noticeable improvement in their condition.

So what is it?

Neurogenx is a breakthrough treatment that uses a combination of medication and electrical signals to heal damaged nerves and restore function. During the treatment sessions, patches with electrodes are placed along your legs. Our experienced Centerville podiatrists then transmit a gentle, wide-frequency electronic signal that’s very similar to electronic waveform pattern used naturally by the body.

Those signals are naturally conducted by the nerves, which help trigger positive changes on the cellular level:

Cell membranes become depolarized, opening their “cellular gateways” so that desperately needed nutrients can get in and waste products and toxins can flush out.

Swelling and pain decrease.

Circulation and metabolism increase.

Cellular healing accelerates.

These electrical signal treatments may be combined with injections of a medical nerve block to provide pain relief. This is especially important because, when a badly damaged nerve begins to heal and start transmitting signals again, temporary increased pain is a common side effect.

Treatment sessions last 40 minutes with 2 sessions per week. The effects are cumulative, and a full treatment course requires 24 total sessions—depending on how your nerves respond. More good news: Neurogenx is non-invasive, non-narcotic, and approved by the FDA. You have nothing to lose and very much to gain.

We’re proud of the amazing results Neurogenx has provided our patients throughout the greater Dayton area, but it's still just one part of a comprehensive approach to neuropathy—and we can help you with that, too. From diet and exercise plans to therapeutic footwear and managing underlying conditions like diabetes, we partner with our patients every step of the way. If you want to not just manage your neuropathy, and not just slow its progression, but actually reverse it, keep it away, and regain at least a portion of your former lifestyle, Sunshein Podiatry is going to give you the best possible chance of getting there.

Treat the Cause, Not Just the Symptoms, of Job-Related Heel Pain

Occupational injuries like heel pain are as common as they are preventable. This foundation is also strong enough to propel our bodies forward as we climb ladders, walk back and forth across concrete floors, and jump in and out of delivery vehicles. In particular, our heels bear most of these stressors as they are usually the first part of our feet to come into contact with ever–varying surfaces. And, as our heels continuously absorb these sudden shocks of impact, pain occurs.

There are also other risk factors you should take into account:

Bearing weight for long periods. This is especially true with hard flooring or surfaces.

Age. The older we are, the more prone we are to experiencing discomfort.

Obesity. If body mass is higher than average, then feet and ankles are also under higher–than–average pressure, which can lead to heel pain.

Ill-fitting footwear. Wearing shoes that provide poor shock absorption and support—especially during high-impact activities—is actually one of the main causes for heel pain.

What's the Best Footwear for You?

You should always opt for supportive shoes, keeping in mind the specific industry requirements that apply to your job:

Is your work environment unusually hot or cold?

Is your work environment often wet or slippery?

Is the ground/surface of your work environment uneven or cluttered with small objects that you inevitably tread upon?

Is the ground/surface of your work environment unusually soft?

Does your work environment consist mainly of hard surfaces, such as concrete, asphalt, wood floors, concrete, and so on?

In some cases, companies will provide their employees with the appropriate footwear according to their assigned roles based on the company’s hazard prevention programs and policies. However, custom orthotics can also be used to complement your work gear and optimize relief from the strains caused by the daily forces exerted on your feet, especially your heels. The American Orthopedic Foot and Ankle Society recommends using custom orthotics in conjunction with simple stretching regimens to significantly reduce heel discomfort and pain.

Need Help? This compromises your foot health in many ways.

Peripheral Neuropathy

Nerve damage caused by diabetes.

Reduces or eliminates the ability to feel pain, pressure, or injury.

Makes it difficult to notice fractures, blisters, or ingrown toenails.

Allows injuries to worsen without awareness.

Diabetic Foot Ulcers

Open wounds that develop due to pressure, friction, or minor injuries.

Often go unnoticed because of neuropathy.

Heal slowly due to a compromised immune system.

Create a high risk for infection.

Peripheral Arterial Disease

A condition commonly associated with diabetes.

Causes reduced blood flow to the feet and lower legs.

Limits delivery of oxygen and nutrients needed for healthy tissue.

Can weaken foot bones over time due to poor circulation.

Charcot Foot

Develops when weakened bones fracture without being felt

Continued walking causes repeated damage and joint collapse

Leads to severe foot deformity over time

Can significantly increase the risk of lower-limb amputation

Recommendations for Diabetic Foot Care During the Holidays

At Sunshein Podiatry, we want you to stay safe, healthy, and continue celebrating for many years to come. Because of this, we place a strong emphasis on diabetic foot care—and you should too. Yes, this time of year can be stressful and even frantic, but for decades, our compassionate podiatrists have provided patients in the Dayton area with expert advice and progressive treatment for diabetes management.

Here are our top tips to give yourself the gift of foot health this holiday season:

Manage your blood sugar levels. Yes, this is something you need to do throughout the year, but you must keep a sharp eye on it during a season that's often associated with sweet treats of all kinds.

Avoid—or at least, limit—sugar in your diet. To connect with our previous tip, you should pass on most cookies and candies at holiday parties, and especially if you don’t know how much sugar they contain. Instead, offer to bring your own desserts or treats. By doing so, you can use diabetes-smart recipes and know exactly what they contain.

Wear your diabetic footwear. When you visit stores and malls to buy presents for loved ones this year, make sure to protect your feet by wearing comfortable diabetic socks and shoes. If you’ve been prescribed a pair of custom orthotics, use them so you are offloading pressure from the appropriate areas of your feet. Remember, at Sunshein Podiatry, we take the time to ensure these medical devices work in conjunction with your unique feet—and this means they make it more comfortable to walk around.

Maintain your exercise program. We know the season is busy, but your health absolutely must be a priority. So, as you plan out your week and determine when to schedule all the chores, activities, and events, make sure to block out time for low-impact exercise first.

Even though diabetes is a serious health consideration, we have some good news: you’re not on your own! You can count on Sunshein Podiatry for optimal foot health. If you need help creating an effective diabetic foot care plan or have recognized that a problem is starting to develop, please don’t hesitate to contact us.

Enjoy More of the Americana Festival Fun With Our Foot Care Tips

Ohio’s largest Fourth of July celebration, the Americana Festival, is a highlight of summer—packed with parades, food, music, fireworks, and fun for the whole family. With tens of thousands of people filling the streets, it’s a day built for walking, standing, cheering, and soaking up every moment of the festivities.

But it’s hard to enjoy the celebration if sore, tired feet start slowing you down. Whether you’re spending the day exploring the festival grounds or lacing up for the Americana 5K Run & Walk, keeping your feet healthy and comfortable makes all the difference. Below, our Centerville podiatrists at Sunshein Podiatry share practical tips to help you stay on your feet, avoid common aches and injuries, and make the most of everything the Americana Festival has to offer—no matter how you choose to celebrate.

Table of Contents

How Can You Keep Your Feet Safe at Outdoor Festivals and Events?

Take a Pass on Flip-Flops

Choose Breathable Footwear

Make Sure Your Shoes Fit Well

Stay Adequately Hydrated

Stretch Before You Head Out

Avoid the Burn

After the Americana Festival, Schedule a Routine Checkup at Sunshein Podiatry

How Can You Keep Your Feet Safe at Outdoor Festivals and Events?

It's worth noting that the following tips aren’t sage advice only for the Americana Festival. You should use them regardless of the type of outdoor festival or fair you and your family or friends plan to attend this summer.

Take a Pass on Flip-Flops

Flip-flops may scream “Summer!” but they’re best saved for short trips or sandy beaches. Most offer little to no arch support or cushioning, and that thin strap between your toes forces your feet to work overtime just to keep them on. Over the course of a long festival day, that extra strain can lead to sore feet, fatigue, and even problems such as hammer toes.

Choose Breathable Footwear

Thankfully, supportive shoes no longer mean sacrificing style. Look for footwear made with breathable materials to help keep your feet cool and comfortable. When paired with moisture-wicking socks, breathable shoes can also reduce sweat buildup and lower your risk of issues like athlete’s foot or toenail fungus.

Make Sure Your Shoes Fit Well

Even the best shoes won’t help if they don’t fit properly. Footwear that is too tight can cause blisters, calluses, and ingrown toenails, while shoes that are too loose can lead to rubbing and instability. Aim for a snug heel, enough room to wiggle your toes, and a fit that feels comfortable right away—no breaking-in required (no matter how cute they are).

Stay Adequately Hydrated

Walking, standing, and celebrating in the summer heat can leave your feet crampy and swollen if you’re not drinking enough water. Staying hydrated helps your muscles function properly and reduces your risk of swelling in the feet and ankles—especially if salty festival snacks are part of the plan.

Stretch Before You Head Out

Even if you’re not participating in the 5K, your feet and legs will still be working hard all day. A few simple stretches before heading to the festival can help reduce stiffness, soreness, and fatigue. Think of it as warming up for a marathon of fun!

Avoid the Burn

If you opt for sandals or open-toed shoes, don’t forget sunscreen—your feet can burn too. Apply SPF 30 or higher, especially during peak sun hours. Supportive closed-toe shoes offer bonus protection from both the sun and the pavement.

After the Americana Festival, Schedule a Routine Checkup at Sunshein Podiatry

If foot or ankle pain is already interfering with your daily life, it’s time to get answers—not just temporary relief. Our team takes the time to thoroughly evaluate your condition, uncover the root cause of the problem, and explain exactly why it developed. From there, we create a personalized treatment plan focused on relieving pain, restoring function, and helping you regain confidence in doing the things you enjoy. Don't be a spectator watching the parade go by—trust us to help you lead it!

Daily Foot Checks: Small Habits That Prevent Big Problems

Your feet carry you through thousands of steps each day, yet they’re often the part of the body we pay the least attention to—until something goes wrong. Subtle changes such as redness, cuts, blisters, or swelling can be easy to miss, but catching them early can prevent minor issues from becoming painful or serious problems. That’s why a simple daily foot check is an important habit for everyone, helping you stay aware of your foot health and address concerns before they interfere with your comfort or mobility.

For people managing diabetes, daily foot inspections are even more critical. Reduced circulation and neuropathy take away your ability to feel pain and abnormalities. This is why regular visits with a diabetic foot care podiatrist are essential for monitoring foot health, preventing complications, and addressing concerns before they worsen. At Sunshein Podiatry, we partner with patients throughout Greater Dayton to protect long-term foot health through education, comprehensive evaluations, and personalized care plans. Our team is here to help you stay one step ahead of potential problems, so here are some recommendations on what to include on your checklist.

Table of Contents

6 Tips for Your Daily Foot Inspection

Sunshein Podiatry: Your Partner in Optimal Foot Health

6 Tips for Your Daily Foot Inspection

When reviewing the condition of your feet, look for anything that seems out of the ordinary. Cuts, scrapes, bruises, blisters, calluses, and ingrown toenails are just a few potential concerns. Whereas a callus might not be a big deal for an otherwise healthy individual, it can become a medical emergency for someone living with diabetes.

We want you to perform at your best because of your vigilance in daily inspections. Here's what to do:

Start a routine. To ensure you check your feet every day, get into the habit of doing your inspection at the same time. Something that works well for many individuals with diabetes is to inspect feet before going to bed every night.

Clean your feet first. Using a soft cloth and warm—not hot—water, gently cleansing before a check. This eliminates the possibility of overlooking potential risks.

Check in a well-lit place. If the lighting in your bedroom is dim, consider inspecting your feet elsewhere. You'll need appropriate lighting to catch even the smallest issues, as tiny problems can still escalate into major issues when left unattended.

Be thorough. It’s not enough to just glance down at the tops of your feet! You need to inspect the tops, bottoms, and between the toes. Review the condition of your nails and also look for unusual growths, such as a plantar wart or callus on the bottom of your foot, or a corn in the webbing of your toes. Any of those can break down over time and become a diabetic ulcer—which is something you definitely want to avoid.

Use your hands. Even if diabetes isn't affecting your eyesight, your hands are invaluable for detecting textual abnormalities. Gently run them all over your feet to see if you feel anything unusual.

Enlist help. If you're unable to see the bottoms of your feet, you need to either enlist the help of a loved one or use a mirror. As mentioned, wounds that become ulcers can happen on all surfaces of the feet.

Sunshein Podiatry: Your Partner in Optimal Foot Health

If you’re noticing changes in your feet, experiencing discomfort, or managing a condition like diabetes, proactive care makes all the difference. The disease has wide-ranging effects on the entire body. As such, it’s easy to lose sight of how diabetes affects foot health. The truth of the matter, though, is that there are serious medical complications that can happen to diabetic feet!

As we explore diabetes and foot health, there are a couple of key areas of concern. You will see that these include neuropathy (nerve damage), peripheral vascular disease, and impaired immune function.

Nerve health is important for living a full life and being able to appreciate everything this world has to offer. They allow you to:

Take in the natural beauty while hiking at any of our local parks;

Catch inspiring performances at the Shuster Performing Arts Center;

Or savor the delicious tastes and smells at the Pine Club (or one of the other fine restaurants in our Miami Valley).

Of course, your nerves do much more than contribute to the experience of living – they actually allow you to live in the first place. As such, it’s quite concerning when elevated sugar levels from diabetes cause damage to nerve tissues throughout the body, especially sensory nerves.

The sensation of touch can certainly contribute to enjoyable experiences—think for a moment about how it feels to hug a loved one or pet the family dog (which also releases stress-reducing hormones!)—but it also lets you know when a problem exists. For this reason, pain can actually be a good thing (as strange as that might be to consider).

It is certainly concerning to lose the ability to feel anywhere on your body. The feet, though, spend a lot of time covered by socks and shoes, plus they are physically the farthest points from your eyes. This means you need to rely on your sense of touch to know when problems develop in your feet. Compromised nerves take away that ability.

Being unaware of existing problems—like cuts, scrapes, ingrown toenails, blisters, and calluses—is further complicated by restricted blood flow. Your body relies on the circulatory system to keep blood flowing, thereby nourishing organic tissue and carrying antibodies to fight off infections. This means unattended wounds do not heal properly and increase infection risk. Over time, this can lead to serious medical complications known as diabetic foot ulcers – a leading cause of lower limb amputation.

We don’t want you to be at risk for diabetic foot ulcers (or any other complication from diabetes), so contact our Centerville, OH podiatrist office and request an appointment. One of our medical specialists can work with you to create a diabetic foot care plan that is based on actions you can take to keep feet healthy and safe. Call us today at (937) 435-7477 for more information or to schedule your appointment.
